I just got done watching Tenjho Tenge volume two and I noticed that Kouji Sagara is no longer played by Lex Lang but in fact George C. Cole. Has this ever happened before besides Excel Saga?

In some of the middle episodes of Record of Lodoss War: Chronicles of the Heroic Knight, Deedlit is played by A. J. Parks instead of Lisa Ortiz.

A big example is "Gundam Wing". Catherine Bloom and Sally Po were both played by Moneca Stori before being replaced by Cathy Weseluck and Samantha Ferris, respectively. And Don Brown started out playing Dr. J before Ward Perry stepped in near the end.

There's the always mentioned change of Zelgadis and Amelia exactly half way through the first season of Slayers from Daniel Cronin and Joani Baker, respectively, to Crispin Freeman and Veronica Taylor. (There was a two year gap between the recording for the first four (1996) VHS volumes and the last four (1998).)

Between Vols. 5 & 6 of Magic User's Club, Akane's VA changed from Nicole Tieri to Stacey Williams.
In A Little Snow Fairy Sugar, Ginger was voiced by a different VA in the first volume. I believe the same for Saga's Mother, who was voiced by Jennifer Sekiguchi afterwards. It was only a short line in first ep., though...
In Rurouni Kenshin, Soujiro's voice changed from Tara Jayne to Lynn Fischer mid-series.
There's a pretty wide gap between any signficant apperances, but Kohaku's voice in Inuyasha changed from Alex Doduk to Danny McKinnon, and Sota and Eri's voice will change from Saffron Henderson to Rebecca Shoichet at some point (don't know when, but has been confirmed).

Oh, since Arxane reminded me of Ocean, there's obviously the big cast changeover with Maison Ikkoku:
Brad Swaile (Godai), Kathy Morse (Hanae Ichinose), Ron Halder (Yotsuya), Anna Cummer (Kozue), Sharon Alexander (Kentaro) and Nicole Bouma (Ikuko) replace, respectively, Jason Gray-Stanford, Daphne Goldrick, Gerard Plunkett, Rochelle Greenwood, Saffron Henderson and Andrea Libman. The first part of the dub running 36 episodes was from 1996 to 1999, the second half started up again once the series hit DVD, starting with Box Set 4 in 2004.

One episode of Cyborg 009 featured David Umansky as 009 instead of Joshua Seth. Also, an episode of Rurouni Kenshin had Tara Jayne replaced by Lynn Fisher for Soujiro.
And then there's Toji from Eva, who went through 3 different voices, ending with Brett Weaver.
EDIT: EmperorBrandon beat me to the RK thing (but I'm pretty sure it was only one episode).

Also, Forstadt replaced Ellen Gerstell as Mihoshi in the Tenchi Muyo! franchise, starting from the second movie through Tenchi in Tokyo, the third film, Tenchi GXP and the new OVA series.
While Rebecca's take on the role is probably closer to the way the character was originally intended to sound and appeal to the audience, I still prefer Gerstell's more mature, intelligent-sounding voice. They both did an excellent job throughout the episodes assigned to them though.

Saber Marionette goes through three different English VA casts over the course of the three "J" series. Each one (J, J Again, J to X) has a completely different English VA cast.
I would love to see Saber Marionette be redubbed one day with a consistent English VA cast for all 57 episodes of the three "J" series.
Another series that had VA changes midway through the series was Sorcerer Hunters with Marron, Gateau, and Big Momma all getting new VA's during the series.

In Ranma 1/2 Venus Terzo replaced Brigetta Dau as female Ranma's voice after 6 episodes and 2 OAVs. Brigetta's vocal performance came off much more tomboyish than Venus' performance but I honestly didn't catch the change until repeat viewings.
The more obvious one that seriously affected the dub was male Ranma's voice being changed from Sarah Strange to Richard Cox (of Inuyasha fame) after the first 3 seasons, the entire OAV series, and movie. While Cox's performance is closer to Kappei Yamaguchi's performance it still lacked the "subtlety" that Strange gave Ranma's voice. Basically Strange did sensitive Ranma better. Cox did jerk Ranma better.

Teryl Rothery voiced her for the first 4 seasons, and the OAVs and movies. Now according to Furinkan.com Sylvia Zaradick voiced here for the remainder of the series. But I swear for Season 5 there was a third voice actress that voiced her specifically for that season.
Jusenkyo Guide was also voiced by Ian Corlett but by Season 7 was voiced by Micheal Donovan(Ryoga). The irony in this being in the Japanese version Ryoga and the Guide had the same voice actor throughout the entire series.
I think that's most of 'em.

CA has it as Erin Fitzgerald (http://www.crystalacids.com/database/actor/fitzgerald_erin.html).
Speaking of Erin Fitzgerald, her roles were replaced in Saber Marionette J by Nicole Oliver because Erin moved to LA. So that counts as a mid-series change for SMJ (the big changes between J, J Again, and J-to-X don't really since they're technically different series).
dragonballz chris sabat did raditz in flashbacks but now raditz is done by justin cook

Different series but same continuity between the three because each series is connected in a linear timeline. Saber Marionette R, on the other hand, while it takes place on the same world, is <span style='color:#dddddd;background:#dddddd'>so far beyond the other 3 series (and has a cast that mirrors the J cast) </span>that it can stand alone with its own English VA cast.

Well since this topic is somewhat fresh and Chris has just done his review of volume #2 of Mermaid Forest (A positive one, might I add), I might as well inform everybody about our own little casting change for the rest of the series.
In Mermaid Forest, the role of Yuta from Vol#2 on will no longer be played by Justin Gross. Unfortunately, Justin became unavailable to record the rest of the series.
However, we are very fortunate to have the wonderful voice acting talents of Liam O'Brien as Yuta for the rest of the series. Liam has done an incredible job as Yuta and I am sure you will enjoy his performance as much, if not more than Justin's.
BUT WAIT THERE'S MORE!
No, I don't mean another VA change, but....SPECIAL GUEST STARS!
Volume#2 will have will special guest star Kyle Hebert.
Volume#3 will have will special guest star Monica Rial.
These are not bit part cameos, these are full episode guest roles. And both have done an amazing job with their roles. I feel extremely honored to have had the pleasure of working with both of these talented people and hope we have the opportunity to do it again.
Oh, there's a couple of other special guest stars in Daphne Vol#4. But it's too early, so I'm not going to tell you /images/graemlins/tongue.gif.

Actually, he was always played by Cole. It was a typo on the part of whoever wrote the cast list.

There was a reason floating around for Mandy's roles being replaced, though I forgot exactly what it was.
Speaking of ADV switches, there's some minor ones in Super GALS! Nora Stein's roles as Minigal Tan and Tan Face Blue were replaced by Serena Varghese and Elena Coates, respectively, in the later part of the series.

Actually, I believe it was episodes 20 and 21. That really bothered me, especially since it was one of my favorite performances from the Lodoss War dubs. Several characters went through similar or complete changes as well--Deedlit's VA change was the most obvious, but Leylia's even more so. I liked Simone Grant the best as Leylia, but for some strange reason, in Episodes 22-24, she was voiced by some Alyssa Beaux (who was nowhere nearly as good as Grant), and, in Episodes 25-27, Meg Frances (slightly better but too whiney).
I learned from Ortiz that her voice change occured because she was out of state and sick at the time these episodes were recorded--and was pretty moritified to hear about the change. I really wondered what was up with some of those other VA changes, though. That was my biggest qualm of this otherwise serviceable dub.

In new episodes Yu Gi Oh!
Bella Hudson replaces Megan Hollingshead (Whos is now in Los Angeles) as Mai Valentine.
Also Mokuba has a New VA as well.
Tara Jayne was replaced by Erica Schroder.
(Erica is the voice of Monkey D Luffy in One Piece)

ANN says the new voice of Mokuba is Caroline Lawson, not Erica Schroder. Tara Jayne was replaced because I think she moved to LA yet again.
And keep in mind that most of the 4Kids VA's come from the same NY talent pool that other studios (particularly NYAV Post) use. There's quite a lot of great VA's among their regulars, so I don't think it's fair to take in out on them for some of 4Kids' more poorly-adapted work.

While I agree she did a bad job on Luffy, to the point where it kind of hurts the ears, Schroder did a WONDERFUL job as Lyserg in 4kids dub of Shaman King. My favorite performance in the cast actually.
