Which anime characters change the most, & most radically in anime?

Is this supposed to be in General Anime?
Also, are you talking intentional personality changes (ie: within the plot) or ones more attributable to bad/inconsistent writing?

Kaede in Shuffle! had me wondering who she would use that large and sharp kitchen knife on first. Or push off the bridge, smother with a hospital pillow . . .

I would say Vincent from Ergo Proxy. Even his appearance changes throughout the series.

What? Character personalities are very consistent across the run of the show, with the changes that occur being relatively subtle, and well within the bounds of what you'd expect given the events at hand.

Another character that goes through a major change would be Masane Amaha from Witchblade. She starts out totally meek, but by the end of the show becomes very strong and accepts her fate.

Yes. Not restricted to R1 releases, & not asking for recommendation particuarly.
Also, are you talking intentional personality changes (ie: within the plot) or ones more attributable to bad/inconsistent writing?

I don’t think there are any spoilers here but let me know if you disagree.
Sven Heartnett in Black Cat. He starts off as a tortured cold assassin and midway through the series suddenly turns into a light hearted bounty hunter with a sense of humour. I personally thought it was bad writing given how sudden the change was but it was clearly intentional.
Marlene in Blue Gender and Eureka in Eureka Seven. Both women start off unemotional and absolutely focused. They are sure of their mission and purpose, which defines them but by the end of the series they are very lively and emotional, willing to give up everything and only sure of the fact that they are in love with the lead character. I was happy with how it happened with Marlene but I had mixed feelings about how Eureka ended up.
Jun Sakurada from Rozen Maiden. In the beginning he was a real pain. He was spiteful, mean, insensitive and emotionally fragile and needed a lot of help to get straightened out. Hell, he needed help to get out of the house! However, by the beginning of Rozen Maiden Traumend, he was emotionally stable, helpful, thoughtful and even counseling Shinku.
Special mention goes to Suigintou who is a moe, pathetic thing in Rozen Maiden Ouverture, A lethal, cunning, powerful evil enemy in Rozen Maiden and then in Rozen Maiden Traumend, gets a bit softer and tsunderey, with the ability to care for others and to have other thoughts besides "Shinku must die!!!"
Wendy Earhart from R.O.D. In R.O.D. Wendy is a bumbling goofball with little self-confidence, barely able to walk 3 meters without falling down. It is amazing that she could be such a high ranking secretary in such a powerful organization. However, in R.O.D. the TV Wendy is coldblooded and efficient, a formidable, intelligent adversary able to intimidate people who are bigger than her with powers that could kill her in seconds. She is a very credible second in command of a global organization. I prefer the second character. I really enjoyed hating her but I really could have done without bumbling Wendy.

I'm confused did you mean to combine Train Heartnet and Sven Vollfied or (the more likely answer) you just meant Train Heartnet?

Simon from Gurren Lagann. He goes from a meek, timid boy with virtually no backbone and nearly falls apart into an emotional wreck after his friend's death to an insanely headstrong and courageous warrior who is able to move on peacefully with his life even after his wife dies. At the end of the day, he's always been Simon the Digger, but he's still definitely grown vastly from the person he used to be.

Couple of characters come to mind. Mashiro of My Otome goes from a spoiled brat who is trying to get her way in things to being someone a queen should be towards the end. The other would be Makoto of Kanon. Starting off as one who has a grudge against Yuichi, well you get the idea.
