Really? So if a character's name is Katou Marika, and someone calls her Katou-san, then Katou would be unacceptable, but Marika-san would be less annoying? That seems backwards to me...

Oh no that's not what I mean at all. Name order flipping is different from given/family replacement (granted those aren't official terms, so I don't know what others call them).
Flipping is Katou Marika → Marika Katou
Replacement is Katou → Marika or Katou-san → Marika-san
Replacement is definitely completely, utterly, outrageously unacceptable but is thankfully rare even in the worst of the commercial translations and virtually nonexistent in fansubs. The LB! BD subs do not do this at all, Naoe-san is Naoe-san.
Flipping is annoying, but since anime characters (in most shows) don't say full names all that much, it's not something that is encountered more than once or twice in a normal episode. It took a decent amount of skimming around to find an example in this volume of LB!, for instance. I'll give heavy preference to subs that don't do this, but it isn't
necessarily a deal-breaker if I have no other options - granted, I usually do have options - unless it's one of those shows where full names come up constantly. (Don't dare touch Ayano's "TOSHINOU KYOUKO!", for example.)
