I've actually never seen this supposedly-returned order combination feature myself either. I never figured out whether it's because my orders weren't eligible, or if it's related to something else. I'm curious if they figure out why it isn't showing up for you, and if that gives a hint as to what all the criteria are.
"I checked your orders and found that some of the items for your orders are * Orders that is restricted a quantity. Therefore, your order cannot be consolidated with other order. I apologize for any inconvenience caused by our order consolidation system."

So the combination system is dumb and can't check if the combination you're doing would actually result in ordering more than one of a restricted item. Figures. Ugh. Time to cancel and place the order again, since they obviously aren't going to add the item for me..

Oh motherfuuuuuuuckers, I cancel my order and get ready to place it again, but it's only after canceling that I notice the Mouretsu artbook is completely out of stock and only available from marketplace sellers already.

Any idea how long it takes an item you cancelled to get re-listed as in stock? And ugh I'll need to catch it quickly even if it comes back. I really hate dealing with Amazon sometimes.

Edit: actually screw it, I'll take the book and CD portion of my order over to CDJapan instead. Less hassle.

Yeah, I think something like that has happened to me before. Either that, or the price went up from the first time I placed the order.

Which is why I always wait until after I've made the new order to cancel the old one.

Yeah, I fully intended to do that but for some reason did things out of order. It also never occurred to me that the Mouretsu Pirates Akiman Design Works book would actually sell out (immediately on release no less, not to mention I forgot it came out today).

In any case, I actually make out about as well shifting 7 items from the order over to CDJapan, or even a little ahead once my frequent shopper points kick. Dropping books off the Amazon order also nixed the ¥1000 penalty which lowers my per item S&H cost slightly over in the Amazon order.

Well this is really obnoxious. I really didn't think that book would sell out immediately. And CDJ is down to 4 copies and I really can't place an order tonight. Well, it looks like AmiAmi has a lot available, so I guess I could always just add it to my already large July order there, but I'm potentially adding significantly to my shipping costs by doing so (but I have no way of knowing due to how shipping with AmiAmi works). Guess I'll just hope it comes back in stock at Amazon by the time I'm ready to make my order in another week or so, and that other items (like Mouretsu Pirates #5) don't go out of stock in the meantime.

Well at least Mouretsu BD comes out a few days later this month, so you have a full week (or I guess 6 days actually) yet before needing to worry about stock on the BDs. I'll be surprised if the artbook comes back in stock though, unless you catch the one I cancelled, if/when that increases the available stock to 1. I still don't see it listed though, and the status isn't even "temporarily out of stock". Of course the marketplace copies are going for more than twice the MSRP (and wouldn't ship int'l anyway).

I've seen things go into the "permanent" out of stock notice ("no longer available from this seller") and come back before. Something like this reeeeeally shouldn't be permanently out of stock unless there's something really unusual about this release. You can usually buy anime/game artbooks looooong after their release date. I have several artbooks for Hanasaku Iroha and some games in my "Save for Later" list, about half from last year, one from 2010, and one from 2009, all of them still in stock and some that are currently at low stock indicate that more are coming. I wonder if books go to the permanent out of stock status when they first go out and then it changes once they know they can get more in stock.

In any case, they often do become available again, but I've never measured or noted how long it took for anything to come back in stock after it went to this status, so I have no idea what a normal time frame for that would be.

It's back in stock. I already placed my order at CDJ a few days ago though. Which is okay, since I had ~2200 points I needed to use and paid for 90% of the shipping.

Default Order history has disappeared

I thought it was just me at first, but then I read this: http://0taku.livedoor.biz/archives/4253638.html

I was looking through my orders last night to update my anime spreadsheet but while I could see open orders and view invoices, the order details page kept coming up blank. Now it seems like just all orders are gone.

Hopefully this gets fixed soon. This is a pretty significant "oops" for a company of Amazon's size.
